The Markham Pickleball Club Board of Directors and Officers declare that the personal information provided will not be shared outside of the stated I purposes.
I, the undersigned, understand that there are hazards (including Covid-19 or any other virus) when playing Pickleball and hereby release and forever discharge the Markham Pickleball Club Board of Directors and Officers, the Pickleball Instructors, and all the Participants of and from all claims, demands, damages, actions, or causes of actions arising or to arise by reason of my participation in any activities forming a part of or in any way related to playing pickleball and from all claims or demands whatsoever in law or equity which I, my heirs, executors, administrators or assigns can or shall have against the Markham Pickleball Club Board of Directors and Officers, the Pickleball Instructors and all the Participants.
I also agree to observe and abide by all the rules and regulations of the City of Markham and the Markham Pickleball Club, and to respect and observe any sanctions that could arise because of a violation on my part of any rules and regulations of the Markham Pickleball Club.
I agree that any pictures taken at Markham Pickleball Club events can be used by Markham Pickleball Club in promotion of Pickleball on the web site or in advertising of Pickleball.
