WAIVER AND RELEASE OF LIABILITY

I, the undersigned participant, acknowledge that I have voluntarily chosen to participate in the North Bay Pickleball Club event held at Bourke Pickleball Courts. I understand that participation in pickleball activities involves inherent risks, including but not limited to personal injury, illness, permanent disability, or even death. I fully assume all risks associated with my participation in this event.

In consideration of my acceptance as a participant in this event, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby agree to:

1. Waive and Release North Bay Pickleball Club, Pickleball Ontario, Pickleball Canada, event organizers, sponsors, volunteers, referees, and any other associated parties from any and all claims, liabilities, or causes of action arising out of or related to my participation in this event, whether caused by negligence or otherwise.
2. Indemnify and Hold Harmless all of the above-mentioned parties from any claims, demands, damages, or liabilities, including legal fees, that may arise from my participation in the tournament.
3. Acknowledge and Accept Responsibility for my physical fitness and ability to participate in this tournament. I affirm that I have no medical conditions that would prevent me from safely engaging in this activity and that I will take full responsibility for any medical expenses incurred due to my participation.
4. Consent to Medical Treatment in case of an emergency, and authorize event staff, medical personnel, or emergency responders to provide necessary treatment. I understand that I am responsible for any medical costs associated with such treatment.
5. Grant Permission for Photography/Videography and understand that photographs or videos taken during the event may be used for promotional, marketing, or reporting purposes without compensation.

By signing, I confirm that I have carefully read, fully understand, and voluntarily agree to the terms of this Waiver and Release of Liability.
