Annapolis Royal Pickleball Club Consent Form

Participant Code of Conduct

By participating in the Annapolis Royal Pickleball Club activities, I agree to the following:

1. Agreement to abide by Annapolis Royal Pickleball Club by-laws
2. Zero Tolerance Policy: Programs held by the Annapolis Royal Pickleball Club are meant to be fun, comfortable and safe for all participants. I understand there will be a zero-tolerance policy for verbal, emotional or physical intimidation or otherwise. If I do not adhere to this policy, I understand that I may be asked to leave the facility by the person(s) leading the program.
3. Sportsmanlike Conduct:
   • I will not engage in unsportsmanlike conduct or encourage others to do so.
   • I will not engage in behavior that endangers the health or safety of others.
   • I will treat others with respect.
   • I will exhibit fairness and honesty in my dealings with others.
   • I will be mindful of manners, politeness, and the feelings of others. Sportsmanlike behavior is required at all levels. I will avoid using profanity, raising my voice, making inappropriate or uncomfortable comments, and making others feel unwelcome.
4. Player Interaction: I will be willing to play with any other player in the session. However, I know I will not be forced to play with a player who makes me feel uncomfortable or unsafe.
5. Coaching: I will not coach fellow players on or off the court.
6. Incident Reporting: Any incident occurring during the session will be reported to the person(s) leading the program, who may take immediate action to enforce the zero-tolerance policy.

Consent and Release

I understand that there are potential risks associated with participating in any program held at the HUB Gym AND I understand that there are potential risks associated with participating in any program held by the Annapolis Royal Pickleball Club. Knowing this, I give my full permission and consent for myself to participate in the program below.

I understand the Annapolis Royal Pickleball Club, the Town of Annapolis Royal and the Annapolis Royal HUB Gym reserves the right to expel a player for any reason with no refund. I understand that Annapolis Royal Pickleball Club, the Town of Annapolis Royal and Annapolis Royal HUB Gym reserves the right to cancel a program at any time with no refund.

By signing the following form, I, my heirs, executors, administrators, and assigns release and forever discharge the Annapolis Royal Pickleball Club and Town of Annapolis Royal, including their respective employees, coaches, and volunteers, from any responsibility, liability, claims, demands, damages, actions, or causes of action arising out of or in consequence of any damage, loss, injury, or contracted illness (including COVID-19), including loss of life, to my person or property incurred while attending or participating in any town course, program, or event. I will not make any claims, demands, or take legal action against them should any of the above occur.

Acknowledgment

I have read and understood the above conditions and agree to abide by them.
