For the Parent or Guardian of a Minor
Pickleball Manitoba Incorporated
Being the parent or legal guardian of the minor child, I hereby consent to Pickleball Manitoba Incorporated (the "Organization") and its authorized representatives using my child's name, likeness, image, portrait, photograph, video and/or audio recordings, statements, and voice in connection with the clinic and related activities.
Permitted Uses include, but are not limited to:
- Photographs, video recordings, and audio recordings
- Social media platforms and websites
- Promotional and marketing materials (print and digital)
- News releases, newsletters, and sponsorship materials
- Broadcast and electronic communications
I waive any right to compensation or royalties arising from such use. I understand that all photographs, video footage, digital files, and reproductions shall be the sole property of the Organization. This consent extends to the Organization's partners, sponsors, contractors, and agents involved in producing or distributing such materials.
I agree to release, defend, and hold harmless Pickleball Manitoba Incorporated, its board members, officers, employees, volunteers, coaches, and any firm it may engage to publish or distribute my child's image(s), from any and all claims, damages, or liability arising from the use of said materials, including any alteration, distortion, or use in composite form, whether intentional or otherwise.
My digital signature constitutes my free, informed, and voluntary acceptance of the full terms of this consent.
